    Several of your points refer to what the industry calls The "User Interface". A good web design firm should have experienced "User Experience Designers" on staff. A User Experience Designer goes beyond the normal graphics design and should conduct structured interviews with the site owners, and hopefully a sampling of ALL potential users to generate user profiles, site goals, and business goals for the site. The User Experience Designer then uses this information in conjunction with their graphical design capability to design a "User Interface" that meets and exceeds all user expectations.
